Make An Appointment?
We are able to see you by appointment only. Please telephone us on (01245) 347539 or call into the surgery personally between 8.00am and 6.30pm Monday to Friday.We also have an online appointment booking
service. Please ask at reception for details. A normal appointment is 10 minutes and is intended for one person. Please make a separate appointment for each person to be seen. If you feel you need more time
then please ask for a double appointment.
There is no restriction as to whom you see in the practice as the doctors provide cover for each other in their absence.
In the event that you need to be seen by a doctor the same day, our receptionist will arrange for you to be seen but not necessarily by the doctor of your choice. Please telephone before 10.00am
where possible.
The doctors and nurses will do their best to keep to time but illness is not always predictable and unforeseen events may cause us to run late, in which case we apologise. Our receptionists will
keep you informed of any delays.
The surgery offers a chaperoning service. Please make your GP, nurse or receptionist aware if you would like a chaperone present at your consultation.

Obtain A Home Visit?
We encourage patients to attend the surgery whenever possible as you will be seen sooner and under more ideal conditions. However, if you feel that you may need a home visit, please contact the surgery
before 10.30am. The receptionist will take some brief details and the doctor will telephone to make arrangements to visit.

Obtain Telephone Advice?
Telephone advice is available from our doctors and nurses. Please ask our reception staff if you require a telephone call and they will be happy to advise on our procedure for this service.

Obtain Test Results?
We give test results out between 2.00 and 6.00pm Monday to Friday and these can be obtained by telephoning the surgery on (01245) 347539 or calling into the surgery in person. To ensure confidentiality,
results will only be given to the patient concerned.

Change My Personal Details?
It is important to let us know of any changes to name, address or telephone number, so that your records are kept up to date. Please include all mobile telephone numbers where appropriate.

Data Protection Act/Confidentiality
We ask you for personal information so that you can receive care and treatment. This information is held in your medical records and recorded on the computer system. Patients need to understand and accept
that information must be shared within the health care team in order to provide their care. This information is protected under the 1998 Data Protection Act to which Whitley House Surgery is registered and is
of course held in the strictest confidence.
Freedom Of Information
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make
available.

How Do I...
Obtain a 'Fit To Work ' Certificate?
the old sick certificate has been replaced by a 'Statement of Fitness to Work' certificate. Under government sick pay regulations no doctor's 'fit to work' certificate is required for illness of seven days or less.
A self-certifying form should be completed if the illness lasts between four to
seven days (including Saturday and Sunday). These are available from your workplace.
If the illness lasts for more than seven days then you do need to obtain a 'fit to work' certificate from the doctor. If your employer requires a doctor's certificate for an absence of less than seven days,
then a private certificate may be issued by your doctor for which there will be a charge.
Fees
Certain services provided by your doctor are not covered by the NHS and for these services you will be asked to pay a fee. Examples include pre-employment, taxi, HGV or private medicals, insurance claims,
holiday cancellation claims etc.
Language Translation
If you need an interpreter for your appointment with a doctor or nurse this can be arranged with Language Line. Please inform reception of your requirement.
